Flat tympanograms can be caused by blockage before the tympanic membrane (e.g. cerumen impaction), pathology of the TM or middle ear (e.g. otitis media), or perforation of the tympanic membrane (in which case, the probe tone reflects off of the middle ear cavity rather than the TM).

Typically with a conductive component in the right ear, one would expect the right ipsi reflexes to be absent and the left contralateral reflexes to be absent because the probe is in the ear with the conductive pathology.
