Medical Evaluation Notes

-The tympanostomy tube is in ear canal, not right eardrum; the eardrum is intact, but there is a recurrence of middle ear effusion.

-The patient was placed in a supine position, his ear drum was numbed with phenol and the canal was cleaned with alcohol.  A circumferential myringotomy incision was made inferiorly; clean serous fluid (no pus or inflammation noted) was removed.  A tympanostomy t-tube was placed in the ear and the patient was given floxin otic solution. He reported immediate improvement in his hearing.
