Case 3 Discussion and Case Management

  • This is an adult with mild hearing loss.  If the ENT physician does not believe that further surgery is appropriate at this time, would you recommend amplification?
  • The loss at 2000 Hz is showing as sensorineural, but this is likely from Carhart’s notch and indicates that when testing by bone conduction, the fixation of the stapes prevents the ossicular lag portion of the bone-conduction mechanism from working.  It does not indicate damage to the cochlea.  If you were to fit hearing aids on this gentleman, would you consider the loss as conductive or mixed?  (Do you expect him to have recruitment, which is associated with cochlear pathology?)  What amplification features are needed, and which are not?
