Case 3 Audiometrics

Where are the air-bone gaps the largest, and where are they minimal?  Note that at 250 and 500 Hz, the unmasked bone conduction threshold, which reflects the “better” cochlea’s sensitivity, is better than either masked threshold.   Why might this occur?

□      Review the mechanisms for bone conduction: compression/distortion of the cochlea, ossicular lag, and bone conduction by air conduction.  If the stapes footplate (or artificial stapes that was previously surgically implanted) is not moving freely, which of these bone conduction mechanisms is impaired?

□       Does the patient hear everyday sounds via air or bone conduction?
